A lot of compelling stories from individual docs here, as well a discussion of the larger picture. Starting your own practice as a healthcare provider is exciting. You can set your own hours, choose your patients/clients, and use the methods and techniques in your field you think work best in your hands. Being your own boss can feel liberating, but it also takes some business know-how. You’ll need to consider factors like finding a location, setting up your office with basic services, payer credentialing, liability, and more.

Finding a Location
Right now, general commercial real-estate is inexpensive, but healthcare office space is still hard to find at affordable prices. It is even harder if you don’t have a full slate of patients yet, and would like to get started practicing part-time. Ideally, you want a space in an area that has a high proportion of patients who carry good insurance, and that is relatively underserved in your specialty. You also want a space with parking, disability access, and amenities that make patients comfortable. MedCoShare has offices in Philadelphia now, and plans to open locations in South Jersey, and Bucks County within a few months.
Regulations and Insurance
Before starting your practice, there are a few regulations you should consider. These include:
Business licenses — You may need to obtain a business license to grant you the right to operate your practice in your municipality. For example, in Philadelphia, every business must apply for a Commercial Activity License (CAL).
Payer credentialing — Health insurance companies formally assess a provider’s qualifications before the provider can bill for their services. This can be time-consuming, and requires the provider to submit extensive background information. In the past, every individual health plan had its own credentialing process, but there are now centralized databases (e.g. CAQH) that allow a provider to submit documentation once, and use it for multiple insurance carriers. The timelines for this should be built into your planning, unless you provide mainly cash pay services (such as cosmetic or wellness procedures) that are not billed to insurance.
Malpractice insurance — Whether you are a physician or a non-physician healthcare provider, you need to get the appropriate professional liability insurance before you start seeing patients. Different types of providers have different insurance needs. To determine what your practice needs, you can connect with your specialty’s national or state professional society. Generally, liability insurance companies cover many types of providers (not just MD specialists, but podiatrists, chiropractors, optometrists, and others), and can advise you on your needs
Operational Needs
In addition to the space itself, you will need to set up various service contracts for your practice. This means basics like utilities, internet, specialized medical waste removal, and cleaning/sanitizing services, but can also mean more specific needs like coding, billing, administrative staffing, and medical assistants. COVID has led to disruptions in many industries, and healthcare is no exception. Telemedicine has been growing in popularity, but the impact is not limited to that. More broadly, healthcare providers have to think more creatively about care models. Here is an article about “hospital at home” care, which is being considered and developed by major players. Independent providers can take advantage of flexible options for care delivery even more nimbly as the healthcare marketplace evolves.
An electronic medical records system (EMR) is a necessity in medical practice today. There are many commercial systems out there, some tailored to the needs of particular specialties, and some more general. Choosing a system is not easy, because the comparison is hard to make truly “apples to apples”. Here is a review of the best systems of 2020, recently published by an independent not connected to any EMR maker.
Most EMRs now live either in the cloud, or are easily installed on a laptop, so you can see patients anywhere and have it with you. The better systems also offer patient scheduling, coding, and billing options.

Co-working has been booming in many industries. Though it started with traditional office work, it has happened with artists, beauticians, and many others. It has gotten to the point where there are websites devoted to it as a concept. Now, one of those websites has discovered medical practice. They didn’t talk to us, but they did a great job of listing the benefits to providers.

The COVID pandemic has led to more physicians seeing patients using telemedicine technologies, helped along by a temporary loosening of restrictions on reimbursement by both government and private insurers. This led to widespread predictions that telemedicine is bound to essentially take over healthcare. This article shows that this may not actually happen. From a physician’s point of view, this is not surprising. There is some outpatient care that simply can’t be done over a video chat (for example, biopsies and minor procedures), and there are patients (especially among seniors) who simply prefer to see their doctor face to face. Some specialties are naturally be better suited to remote practice, and others (more procedure-oriented) will be less so.
To whatever extent telemedicine becomes more widespread as a result of COVID-19, the use of office sharing makes sense in parallel. If a physician (or other provider) shifts to a combination of telemedicine and in-person care, they may want to split their time. For example, an endocrinologist might see patients by telemedicine three days a week (when they need only to review labs and medications), and in person two days a week (for physical exams, thyroid biopsies, etc.). Right to request deletion of the data – Request to delete
You can ask for the deletion of your personal information. If you ask us to delete your personal information, we will respect your request and delete your personal information, subject to certain exceptions provided by law, such as (but not limited to) the exercise by another consumer of his or her right to free speech, our compliance requirements resulting from a legal obligation or any processing that may be required to protect against illegal activities.

Verification process
Upon receiving your request, we will need to verify your identity to determine you are the same person about whom we have the information in our system. These verification efforts require us to ask you to provide information so that we can match it with information you have previously provided us. For instance, depending on the type of request you submit, we may ask you to provide certain information so that we can match the information you provide with the information we already have on file, or we may contact you through a communication method (e.g. phone or email) that you have previously provided to us. We may also use other verification methods as the circumstances dictate.
We will only use personal information provided in your request to verify your identity or authority to make the request. To the extent possible, we will avoid requesting additional information from you for the purposes of verification. If, however, we cannot verify your identity from the information already maintained by us, we may request that you provide additional information for the purposes of verifying your identity, and for security or fraud-prevention purposes. We will delete such additionally provided information as soon as we finish verifying you.

you may request correction of your personal data if it is incorrect or no longer relevant, or ask to restrict the processing of the data.
you can designate an authorized agent to make a request under the CCPA on your behalf. We may deny a request from an authorized agent that does not submit proof that they have been validly authorized to act on your behalf in accordance with the CCPA.
you may request to opt-out from future selling of your personal information to third parties. Upon receiving a request to opt-out, we will act upon the request as soon as feasibly possible, but no later than 15 days from the date of the request submission.
